Home Rehabilitation
The home rehabilitation unit carries out an assessment and rehabilitation period for seniors, whose independent capacity and management of day to day chores have deteriorated when other services or support measures no longer are sufficient for supporting their living at home. Rehabilitation at home allows efficient assessment and rehabilitation in connection to daily routines to take place at an individual’s own home.
During at-home rehabilitation, a senior citizen is given guidance for practising their day to day routines and mobility, to keep up with their changing capabilities and capacity.
Extended Hospital Care
Services for extended hospital care allow
- return home supported by the extended care hospital
- rehabilitation taking place at home, e.g. the rehabilitation provided after acute treatment, or temporary rehabilitation required by a home care client
- medical care taking place at home and care home
- terminal care taking place at home and care home
